ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the comparability and bias of the test results of two detection systems for serum procalcitonin (PCT) under the same laboratory condition.</p><p><b>METHODS</b>According to the profile NCCLS-EP9-A, the two systems were used to detect PCT to obtain the correlation coefficient and the liner equation for evaluation of the test result bias.</p><p><b>RESULTS AND CONCLUSION</b>The test results of PCT showed no significant difference between the two detection systems (P>005) with a kappa value greater than 0.75. The correlation coefficients of both systems were above 0.975, suggesting a consistency between them for clinical detection of PCT.</p>

ABSTRACT

Objective To investigate the efficacy and safety of intrathecal chemotherapy combined with concurrent radiotherapy in patients with leptomeningeal metastases from solid tumors.Methods The clinical and follow-up data of 29 patients with leptomeningeal metastases from malignant solid tumor who had intrathecal chemotherapy combined with concurrent radiotherapy were retrospectively analyzed.The treatment regimen was that 12.5-15.0 mg of methotrexate intrathecal injection once a week for 8 successive weeks combined with whole brain irradiation to a total dose of 40 Gy,20 fractions or with lumbosacral spinal canal irradiation to a total dose of 50 Gy,20-25 fractions.Results Nineteen patients completed the concurrent therapy and 19 patients were dead.There were 9 patients with complete remission of symptoms,11 with obvious improved symptoms,6 with alleviated symptoms,and 3 with no relief.All of the 29 patients,were followed up for 0.4-15.0 months.The median survival was 5 months.The 6-month survival rate was 48% (11/23),excluding 4 patients with follow-up time less than 6 months and 2 patients noncancer deaths.The main toxicities were myelosuppression,methotrexate-related mucositis,chemical nerve root injuries,chemical meningitis,chronic neurotoxicity,seizures,and radiation-related mucositis.The incidence of severe toxicity was 17% (5/29).Conclusions Combination of intrathecal methotrexate with concurrent radiotherapy can effectively alleviate the symptoms and improve the life quality of patients.The therapy with low incidence of severe toxicity and good tolerance has the trendency to prolong the median survival.

ABSTRACT

Leptomeningeal metastasis (LM),with a high omission rate in diagnosis in approximately 5 %~8 % of all patients, is increasingly recognized as a lethality complication. The diagnosis of LM remains challenges,but commonly can be established by CSF cytology or by definitive neuroimaging. Although a combination of focal radiotherapy,intrathecal chemotherapy,and systemic chemotherapy may be necessary to achieve optimal treatment of patients, but prognosis is unacceptable and therapy remains palliative in most patients.

ABSTRACT

Objective To evaluate the efficacy of ultrasound ablation and balloon angioplasty in treating peripheral arterial occlusion in diabetes mellims. Methods One hundred and fifty-eight patients with peripheral arterial occlusion were treated with the ultrasound ablation. Through percutaneous approach or surgical approach, the ultrasound ablation catheter was inserted into occluded arterial segments. Results The ultrasound ablation was successful in 149 of 158 patients (94%). After treatment, the average skin temperature rised for 3℃ and the distance of intermittent claudicafion was increased for 1 000 m. The gangrene was mitigated in 38 of 45 patients. The pain during rest was ameliorated in 31 of 35 patients. Long-term efficacy was 75%. Conclusion Ultrasound ablation is effective and is a new approach for treating peripheral arterial occlusion in diabetes mellitus.

ABSTRACT

Objective To genotype strains producing ESBLs in Escherichia coil and Klebsieua pneumoniae to provide references for the clinical application of drugs.Methods PCR was used to determine the genotype of the ESBLs.Results The percentage of TEM type,SHV type and NO-TEM-NO-SHV type in Escheroichia coil was 80.4%,7.8% and 11.8%,in Klebsieua pneumoniae was 78.1%,71.9% and 25.0%.Most of Klebsieua pneumoniae produced more than two Extended Spectrum ?-lactamases.Conclusion The difference in the distribution of genotypes in different regions is confirmed.

ABSTRACT

Objective To evaluate the clinical efficacy of ultrasonic ablation in patients with chronic deep vein thrombosis of lower extremities. Methods Fifty-six patients with chronic deep vein thrombosis resulting in total occlusion of iliac-femoral veins accepted intra-venous ultrasonic ablation. The time for ultrasonic ablation was 12-24 minutes with the mean being 18 minutes. After the procedure, anti-coagulation was performed through the retaining catheters and venous propeller was utilized for promoting blood reflow. Results Ultrasonic ablation easily created a channel within the occlusive iliac-femoral veins and achieved successful recanalization in 49 of 56 patients (87.5%). Five patients (8.9%) were partially re-canalized and 2 (3.6%) had no recanalization. Thirty-eight of the 49 re-canalized patients were followed up, including 22 with no stent placement showed 9 (40.9%) re-occlusion and 16 received stent placement, and 2 had (12.5%) re-occlusion. Conclusions Intravascular ultrasonic ablation is an effective therapeutic modality to recanalize chronic thrombosed occlusion of iliac-femoral veins and can lay down a foundation for further balloon-expansion or stent-placement with long term patency.

ABSTRACT

Objective To investigate the efficacy of ultrasound ablation combined with autologous transplantation of bone marrow mononuclear cells in treatment of arteriosclerosis obliterans.Methods Clinical data of 39 patients with arteriosclerosis obliterans treated with ultrasound ablation and autologous transplantation of bone marrow mononuclear cells were analyzed.There were 21 male and 18 female patients,With average age of 63 years.At operation,260 mL of autologous bone marrow was withdrawn,mononuclear cells were separated,and 40 mL supension fluid of stem cells was prepared.Through percutaneous approach or surgical approach the ultrasound ablation catheter was inserted into occluded arterial segments.After ablation was performed,autologous bone marrow mononuclear cells were directly injected into the skeletal muscles of the ischemic leg.Results No complications were found in this group.Postoperatively,rest pain of all the patients was significiently improved and distance of claudication increased.The skin became warm in all legs.Conclusions Ultrasound ablation and autologous transplantation of bone marrow mononuclear cells is a simple,safe and effective method for treating patients with arteriosclerosis obliterans.

ABSTRACT

Objective To investigate the effects of hexamethylene bisacetamide(HMBA),3-(2-hydroxy-3-methyl-butyrylamino-)-propionic acid(HMBPA) and HMBA plus HMBPA on MELDS19 cell growth and differentiation.Methods To make the MELDS19 cell growth curve,count the MELDS19 cell mitosis index,numbers of colony forming and benziding staining positivity percentage when MELDS19 cells exposed to HMBA,HMBPA and HMBA plus HMBPA.Results The numbers of the MELDS19 cells exposed to 2mmol/L HMBA was 5.0?10~5/ml,to 1?mol/L HMBPA was 6.0?10~5/ml and to 1?mol/L HMBPA+2mmol/L HMBA was 3.3?10~5/ml respectively,mitosis index and colong forming decreased.Benzidine staining positivity percentage exposed to 2mmol/L HMBA was 38%,to 1?mol/L HMBPA was 9% and to 1?mol/L HMBPA+2mmol/L HMBA was 64% respectively.Conclusion HMBA and HMBPA can inhibit MELDS19 cell proliferation and induce differentiation much effectively than HMBA or HMBPA used alone.
